all high school rugby events in which your
team is participating (i.e. practices and
games). In each notebook you should have
these 4 Player Forms:
A. Signed parent consent forms from each
player.

Results must
be submitted within 48 hours following
game day.
The form must be filled out in its entirety.
You can download a PDF version that you
can print out to use on game day. All results
will be posted as soon as possible.
